J Visa Compliance

J VISA*

Seven Corners has created an insurance plan specifically designed for the J Visa dependent(s).

Schedule of Benefits

Medical Maximum: $50,000 Medical Maximum is per Person per Occurrence
Deductible Options: $0; $100; $225; $500 Deductible per Person per Occurrence
Coinsurance Options: After you pay the deductible, the program pays 80% of the next
$5,000 of eligible expenses, then 100% to the selected Maximum
Dental (Emergency): $100 (or $500 for accidents)
Emergency Medical Evacuation/ Repatriation: $25,000 (in addition to the Medical Maximum)
Return of Mortal Remains: $10,000
Emergency Reunion: $10,000
Return of Minor Child(ren): $5,000
Local Ambulance Expense: $2,500

 


Eligibility
Eligible Participants shall be J-1 Visa Holders, J-2 Visa Holders, and their Dependents who are temporarily residing outside their Home Country, and whose name and travel dates have been submitted and have been accepted by the Administrator. The J-1 Visa Holder must remain engaged in educational or research activities outside their Home Country during the Period of Coverage. The J-2 Visa Holder and/or their Dependents are eligible as long as the related J-1 Visa Holder remains in official status.

Dependents are considered to be the Primary Insured Person's spouse and natural or legally adopted unmarried children over 14 (fourteen) days and under 21 years of age while traveling outside of their Home Country. Home Country is defined as - The country where an Insured person(s) has his/her true, fixed and permanent home and principal establishment. Coverage shall include the United States.

Pre-Existing Conditions
For Medical benefits, this insurance does not cover:

Any Injury or Illness which meets the following criteria: (a) condition(s) that would have caused a person to seek medical advice, diagnosis, care or treatment during the 12 months prior to the Effective Date of coverage under this Policy; (b) condition(s) for which manifestation, medical advice, diagnosis, care or treatment was recommended, received, or noticed during the 12 months prior to the Effective Date of coverage under this Policy. This waiver does not include coverage for known, scheduled, required, or expected medical care, drugs, or treatments existent or necessary prior to the effective date of this program.
